The Ohio Cardiovascular and Diabetes Health Collaborative (Cardi-OH) is hosting a webinar on May 22 from noon to 1 p.m. titled “Social Determinants of Health: Implementing Screening and Taking Action.” Keynote speaker Rachel Gold from Kaiser Permanente Center for Health Research and OCHIN will discuss understanding barriers and facilitators to the implementation of screening for and making referrals to address social determinants of health in primary care settings, as well as learning about current evidence on strategies for decreasing the health impacts of social risk.

As a statewide program, Cardi-OH includes investigators from Ohio’s medical schools and is based within the Center for Clinical Investigation under Community Health Programs at Case Western Reserve University School of Medicine. Founded in 2017, the mission of Cardi-OH is to improve cardiovascular and diabetes health outcomes and eliminate disparities in Ohio’s Medicaid population.
